For UVA1 treatment, first contact your scleroderma expert to see if it they will recommend it for you. If they recommend and prescribe it for you, try to find UVA1 facilities near you. Some dermatology offices (usually large offices or with major health centers) have UVA1 equipment (not UVB). It may also be possible to buy UVA1 phototherapy equipment for home use. It is manufactured by Daavlin and distributed worldwide. Home users must be monitored by a physician, who will prescribe the necessary dosage and monitor for possible side-effects, such as skin cancer.
A case of cutaneous lichen sclerosus et atrophicus (LSA) effectively treated by extracorporeal photochemotherapy (ECP). The fact that ECP is effective in LSA, graft-versus-host disease (GVHD), erosive lichen planus, and scleroderma strengthen the hypothesis that there is a common link between these four conditions. Wiley. Photodermatology, Photoimmunology & Photomedicine, 7 May 2013. (Also see Graft-Versus-Host Disease)
Clinical evaluation of vulvar lichen sclerosus: case series. The first-line treatment is topical-potent or ultra-potent corticosteroids in the treatment of lichen sclerosis. Vulvar lichen sclerosis may be associated with autoimmune and thyroid diseases. Kazandi M. (PubMed) Eur J Gynaecol Oncol, 2010;31(5):555-8.
Treatment of oral lichen sclerosus with 1% pimecrolimus cream. We report on a 7-year-old Korean girl with a well-demarcated 1.2x1.2 cm atrophic white plaque with an erythematous border and focal telangiectasia on the left vermillion lip, extending to the labial mucosa. Kim CY. (PubMed) Ann Dermatol, 2010 Aug;22(3):326-9.
Lichen sclerosus (LS): treatment and follow-up at the departments of gynaecology and dermatology. Although no hospital guidelines existed, management of patients with LS agreed with current recommendations in the literature, although differences in secondary and supportive therapy existed owing to differences in expertise. The relatively high percentage of patients treated by both specialties with a high frequency of visits emphasizes the need for a multidisciplinary clinic for vulvar disease. van der Avoort IA. (PubMed) J Low Genit Tract Dis, 2010 Apr;14(2):118-23.
Efficacy of UVA1 phototherapy in 230 patients with various skin diseases. Good therapeutic effects of UVA1 therapy were shown in patients with atopic eczema, scleroderma, lichen sclerosus et atrophicus, keratosis lichenoides chronica, prurigo nodularis and with cutaneous T-cell lymphoma. Psoriasis and concomitant fibrosing disorders: Lichen sclerosus, morphea, and systemic sclerosis. In this population, a predisposition toward autoimmunity is seen as 38.5% of patients had a personal history of a third concomitant autoimmune disease, in addition to psoriasis and fibrosing disorder, whereas 42.3% reported a history of a first-degree relative with an autoimmune disease. Journal of the American Academy of Dermatology Volume 67, Issue 5 , Pages 1079-1083, November 2012. (Also see Psoriasis, Morphea, Systemic Sclerosis, and Multiple Autoimmune Syndrome)
Epstein-Barr virus (EBV) and human papillomavirus (HPV) infection in vulvar lichen sclerosus (LS). Our results showed no relationship between HPV and LS. This result is in accordance with the literature. We have found 26.5% of EBV in our samples. This is a preliminary study, and the follow-up of these patients will elucidate whether EBV could play a role in cases of LS. Aidť S. (PubMed) J Low Genit Tract Dis, 2010 Oct;14(4):319-22.
National Lichen Sclerosus Support Group of the United Kingdom. Established in 1997, we provide support on an international level. It is not known what causes LS but it has been found that there is a connection between LS and thyroid disease, vitiligo and other autoimmune diseases. NLSSG
